Registering as a new patient

You will need to complete the online registration form to register at our surgery. This form will ask questions including details of your previous GP surgery and basic health and medical information.

Although it is not mandatory, you may be asked to provide a copy of your ID for a proof of an address. For example, valid ID could include a photograph of your passport or driving licence. This is to check that you live in the local catchment area.

To find your nearest Adderley Green Surgery and to register as a new patient, go to the NHS Find a GP website and click 'Register Online with this GP' to complete the registration form.

We will require a completed form for every member of your household who wishes to register. The application may take a few days to complete. If you have not heard back from the surgery within 5 working days, please contact the surgery by phoning 01782 311266 or email adderleygreen@stoke.nhs.uk

Non-English Speakers 

These fact sheets have been written to explain the role of UK health services, the National Health Service (NHS), to newly-arrived individuals seeking asylum. They cover issues such as the role of GPs, their function as gatekeepers to the health services, how to register and how to access emergency services.

Special care has been taken to ensure that information is given in clear language, and the content and style has been tested with user groups.

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

If you are away from home, you can register as a temporary patient with Adderley Green Surgery for up to three months. This will allow you to use our surgery and still remain a patient at your permanent GP surgery. After three months, you will have to re-register as a temporary patient or permanently register with us.

Register as a temporary patient

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered. 

Children under 16 need to be registered by their parent or guardian. You can do this by completing the new patient registration form, the GSM1 form and child immunisation form.

You will need to provide a form of ID to prove that you’re their parent or guardian.

Once completed you can hand in to the surgery or you can email to Adderleygreen@stoke.nhs.uk

If you are a carer of young children, elderly adults or adults who aren’t able to register themselves, you can set up an account on their behalf. This will allow you to book an appointment and access parts of their GP online services account.

If you wish to pre-register click on the link below to open the forms. When you have completed all of the details, click on the "Submit" button to mail your forms to us. When you visit the surgery for the first time you will be asked to sign the forms to confirm that the details are correct.

If you are moving house, please let us know as soon as possible. If you are moving out of the surgery’s catchment area, you may be asked to register with a surgery that’s closer to your new home address.
